Telephone headsets

Hello, I'm hoping someone here can share some advice on headsets.

I wear a binaural telephone headset at work.  Nothing fancy, but it's been a real neck-saver for me.  Since my hearing started to change I'm finding that my old headset doesn't meet my needs anymore.  The "workplace accomodation" people are willing to get me a new headset, but I have to tell them what I need.

There are basically two problems I need help with.  First, I need to somehow screen out background noise on the callers end.  When someone calls from a noisy environment, it's like trying to hear one instrument in a marching band.

The other problem is with the "shouters".  If I turn the volume on the headset down so it's at a reasonable volume in my good ear, I can hardly hear them in my bad ear.  Ideally, I'd like a headset with separate volume controls for each ear.

Any suggestions?
