Boston AN Pain Specialist Dr. Acquatro

Hi All, Anyone know Dr Acquatro? Works out of Carney Hospital in Dorchester. Went to see Dr. Barker @ MGH due to muscle spasms in my neck that spread to my head. Told me I should not be experiencing this level of pain 8 mo. post op. Referred me to Dr Acquatro. I see him Monday. Just wanted to know if anyone has had any dealings with him? Also if there is anyone else specializing in neck/headaches I could see in the Boston, Worcester Prov area? Janet has offered to school me in what to ask and look for.  ;DThanks Joe
Retromastoid Mass Gen 7/30/08 Drs. Barker & Gopen
2.5 x 3.4 12hrs completely removed. 100% facial palsy but facial nerve intact.
75% return of movement as of 8th month, removed eye ingot 3/09 good progress.
Deaf R side, tinnitus, balance problems, muscle spasms, muscle tension headaches daily.

Hey Joe,

My pain mgt doc (another Brit in my life!) is based out of Beverly Hospital with an affiliation with Brigham/Woman's.  My NS at the Brigham is also following me for headache issues (due to my Chiari 1, etc....)  and referred me to Faulkner.  I'll be seen at the Faulkner Headache Clinic in 2 wks.  The Faulkner has a few locations. I have to drive to Chestnut Hill for my appt.

I'm sorry I don't know of your doc at Carney but happy to PM you my info, if you need it.

Hi Phy,

Thanks for the info. I'm on the cape so anything on the south shore would be great. My email is on my profile so drop me your info. I'd like to hear more about your issues and the Faulkner clinic. Headed up to MEE this afternoon for PT on my face. There were talking about Botox but I don't see why?? Anyway maybe they see something I don't.....whatever. I wish they were closer it will be the second time this week downtown and it takes up my whole day. Thanks again. Joe

Hi Joe,

I had surgery 3/08 at MGH, Dr. Martuza.  Began suffering from headaches, muscle spasms few weeks after surgery.  I have not had much luck with MGH recommending post-op treatment options.  Went to Tufts Craionfacial Pain Center for several months, that has been interesting.  Finally got them to recommend a Headache Neurologist, Dr. McGeeney, saw him today, had a good meeting, we will see how it goes with his treatment plan.  A medical secretary at MGH mentioned Dr. Acquatro to me, but had no idea where he was practicing.  Curious to hear what you thought if you wouldn't mind sharing.

Hi Mary Beth,

Sorry to hear you are having similar problems I pray we both find a solution. Went to see Dr Acquadro today and he was confident he could get this under control. He deals w/lots of AN patients. He said my case was very manageable. He seemed very confident that he could treat this with Botox, Lyrica and a Lidoderm patch.

He sent me to Cambridge to see a Dr. Gary Borodic from Harvard Med School and MEE. He gave me three shots of Botox right under my incision which is where my spasms give me the most problem. He said he would start with the right side and see how that goes. I get spasms on both sides but he wanted to wait and see how this goes. I have a follow up in three weeks. He said it won't even start to have any effect for three weeks anyway. Cost me $600 and Harvard Pilgrim probably won't cover it.....ouch. I also feel like crap. They told me I might not feel so good.

He also put a plug in my eye to keep it from draining. My eye dries out especially on windy days. I use the Refresh w/Celluvisc. I see double out of my right eye.

They probably won't cover the Lyrica either. He said it was like Neurotin only much cleaner w/less side effects but it was a new drug and expensive. They'll pay for Neurotin but not Lyrica. One of those catch 22's I guess. I'm calling and I'm going to give it a go and see if they'll pay for both. We shall see.

Both seemed to think they just have to get the spasms under control. They said it was a combination of things. The cutting of nerve, muscle etc as well as being in a halo for 12 hours getting a hole drilled in my head. They said it is a vicious cycle and they just have to break the cycle which will eventually help me return to normal. I hope they're right otherwise I'm going to start exploring a surgical fix like Janet opted for. I'm about done with this drug thing. I'll give this a few months and see what happens. The only way I’ll know is coming off meds completely which is my goal. I'm tired of being groggy, dizzy and harming vital organs......I'm going to need them for a whole lot longer hopefully.

If you have any questions at all feel free. Keep me apprised of your situation and we'll compare notes. I don't have classic migraine symptoms. My biggest problem is spasms that lead to muscle tension headaches. I think I'm having some rebound headaches that feel like hangovers but they're mild enough I can deal with those for now and ramp down on the OTC's. He did say there was nothing alarming about the amounts I'm taking. I take the minimum to keep me going.

Anyway I'm going to ramp up the Lyrica over a few weeks and try and wean myself off of the Tylenol, Naproxen, Aspirin, Ibuprofen, Cyclobenzaprine and Diazepam .....I'm now taking, yikes. I'll keep you posted.

I'm liking this Botox. It worked on my muscle spasms immediately. Although my headaches didn't go way, the daily spasms just under my incision did which triggered a lot of headaches if I didn't stay ahead of it.

Had a total train wreck on the 16th. Woke with a 10 at 2am and a 9 at 6am. That has never happened before w/two break through headaches hours apart. Anyway took out the heavy meds and was back to sleep in an hour but what an hour it was....yikes. My new bedtime cocktail is sweet; Excedrin Migraine (Acetaminophen 250mg, Aspirin 250mg, caffeine 65mg) w/1/4 Diazepam ¼ Cyclobenzaprine and 75mg of Lyrica.

Dr. Feel Good loves Lyrica. Not sure if it's working yet I only started on it the 18th. They said it takes a couple of weeks to kick in.  I haven't had any severe headaches since going on it but it’s only been a few days. It’s like Neurontin but w/o all the nasty side effects which I have none of so far but the dosage is going to be ramped up. It is an anti inflamatory for nerves. Dr Acquatro has had a lot of success treating AN patients with it.

So we shall see, so far so good. I'll keep you posted. I'll slowly wean myself off all the other meds if it works.

I am a patient of Acquadro ... I had surgery March 08; started pain mgmt August 08.  After 8 months of treatments with him I am beginning to feel results.  I have less general pain, but I still have real pain on the right side. If I stay quiet I am much better; but  I am not able to do anything that requires effort,  bending over is the worst ... and I really fear Respiratory infections - coughing/sneezing are painful.  Luckily I am retired, so that I can spend my day quietly.  But that gets boring too.  I really like Dr. Feel Good - he is generally very encouraging; has managed many AN paitients so he has seen the other side.  He constantly tells me that I will get better over time - but the meds will help me lose the pain sooner.   I am also very intrigued by Janet's surgery solution - I hope it does not get to that for me though!  good luck all - you have sympathy & cheerleaders here.
