Official Survey for AN and Fatigue (survey in document form)

Hi All,

Well, I received the official survey form from Dr. Medbery.  It is a very easy form to complete that will take less than 5 minutes.  If you are interested in doing the survey, please let me know here and I will email it to you (need to email off site as it's a document attachment).  Dr. Medbury's comments below from the email when he sent me the document:

"Let me know if there are any problems with this. People can either mail me the form or fill it out and send it electronically. Tell them to be careful about putting acoustic neuroma or something similar in the subject so I can quickly recognize it is not spam. They can send it to this address"

Please let me know if you want a copy of the survey so we can get it back to Dr. Medbery... saying "thank you" to him just doesn't seem to justify my aprpeciation for his help.

BTW, survey is geared to all AN patients, regardless of treatment type.

Send me the survey please.  Fatigue hit me this afternoon out of the blue - just felt like going to sleep suddenly.  Luckily it only lasted for a few minutes.  Strange how it comes and goes...  There's got to be some biological explanations.

Mary, do I have your internet email?  If not, please email me here with it.

The survey is a VERY small MS Word document, less than 1 minute to download, even on dial up.
