left vestibulocochlear schwannoma
« on: December 17, 2018, 04:11:35 pm »
49year old  male diagnosed in December 2017.Needed second opinion on left vestibulocochlear schwannoma measuring 1.4 x 1.2 x 1.4 cm abutting the left middle cerebellar peduncle. Seeing Dr Mattox in Emory.
6 month MRI shows a change as follows:

FINDINGS: On high-resolution imaging through the enhancing mass centered in the left cerebellar pontine angle cistern extending across the porus acusticus into the medial aspect of the internal auditory canal measuring approximately 1.4 x 1.2 x 1.4 cm
(axial long axis by axial short axis by craniocaudal) is slightly increased in size from prior studies (measuring approximately 1.1 x 0.8 x 1.0 cm on study dated 12/11/2017). The mass abuts the left middle cerebellar peduncle but no significant mass
effect or underlying parenchymal edema is demonstrated. Additionally, no lateral extension to the cochlear aperture is demonstrated. No other abnormal enhancing mass or lesion is demonstrated.

The right vestibulocochlear nerve, right facial nerve, bilateral cochlea, bilateral vestibules, and bilateral semicircular canals are normal. No enlargement of the vestibular aqueduct is demonstrated. The mastoid air cells and middle ear cavities are

A few small scattered foci of T2 hyperintensity in the cerebral white matter are unchanged from prior study and entirely nonspecific but likely related to either mild chronic microvascular ischemic changes, gliosis from old inflammation, chronic migraine
headaches, or even old trauma. Otherwise, the brain is normal in configuration and signal intensity. No abnormal restricted diffusion to suggest an acute/subacute infarction is demonstrated. No abnormal susceptibility artifact is demonstrated. No
hydrocephalus or abnormal extra-axial fluid collections are present. Flow-voids in the larger vascular structures are normal.

Minimal paranasal sinus mucosal thickening is essentially unchanged. The mastoid air cells and middle ear cavities are clear. Marrow signal is within normal limits.
