Title: Official Survey for AN and Fatigue (survey in document form)

Hi All,

Well, I received the official survey form from Dr. Medbery.  It is a very easy form to complete that will take less than 5 minutes.  If you are interested in doing the survey, please let me know here and I will email it to you (need to email off site as it's a document attachment).  Dr. Medbury's comments below from the email when he sent me the document:

"Let me know if there are any problems with this. People can either mail me the form or fill it out and send it electronically. Tell them to be careful about putting acoustic neuroma or something similar in the subject so I can quickly recognize it is not spam. They can send it to this address"

Please let me know if you want a copy of the survey so we can get it back to Dr. Medbery... saying "thank you" to him just doesn't seem to justify my aprpeciation for his help.

BTW, survey is geared to all AN patients, regardless of treatment type.

Phyl,

Send me the survey please.  Fatigue hit me this afternoon out of the blue - just felt like going to sleep suddenly.  Luckily it only lasted for a few minutes.  Strange how it comes and goes...  There's got to be some biological explanations.

Mary, do I have your internet email?  If not, please email me here with it.

The survey is a VERY small MS Word document, less than 1 minute to download, even on dial up.

I honestly don't know what to say. :)

Dr. Medbery's inbox is going to be flooded with the terrific responses of interest for this survey. I want to personally thank you for your help with this.

I have to apologize to everyone  that has not received the survey yet. I just got back in town and will send them this week... sorry all... been a hectic (and VERY tiring!) week for me.

Ok, I think I got caught up on everyone that requested the survey... and if I missed you, please note here that you  didn't get the survey so I can send along to you.

Your overwhelming responses to help do this is so appreciated.... and it's your pro-activeness (is that a word?) in helping us with this is amazing... thank you all.

Please send me the survey. <bobpoe331@comcast.net>. I am tired all the time . 3 months post op. Left side face and ear. Left eye stays open and no tears. But the fatigue is really bad. I can get up in the morning and a few hours later I want to lay back down.   Bob

Hi all,

First, let me preface by saying thank you to ALL of you that have helped with participation in this Fatigue Survey. Dr. Medbery noted that he has received 22, thus far, and we appreciate your help.

We need more participants for this survey, so if you have not completed one and would like to, please let me know here. You can send me a private message (see the PM under my name/picture here to the left) and provide me an internet email and I'll be glad to forward one along.
