Title: Cochlear offers free service regarding BAHA
Post by: wendysig on November 23, 2008, 03:04:54 pm
I thought I'd pass the following information along to anyone interested in getting insurance coverage for BAHA.  My doc told me Cochlear offiers a free service to doctors who wish to help their patients obtain insurance coverage for BAHA.  This is done through Otologic Management Services, a subdivision of Cochlear.  The doctor and patient will have forms to fill out and must present supporting  documentaiton to show the need for BAHA.  The patient has to fill out a release form and supply a copy of the front and back of their insurance card.  OMS makes no guarantee that coverage will be obtained but works directly with the insurance company to try to obtain coverage.  I am not able to copy the information the doctor gave me here, but Cochlera's address, phone # and fax # follow:

Cochlear Americas Corp.
400 Inverness Parkway, Suite 400
Englewood, CO 80112
Phone # 1-800-633-4667
Fax # for OMS (303) 524-6765

Hope someone finds this of some use.

Wendy -

I've never heard of this, but it's awesome.

I'm hopeful that this will help lots of others - Soundy comes to mind  :)

Thanks for posting this information,

Cochlear is helping me with my appeal for the BAHA. They faxed me all the paperwork. I wrote a personal letter to Anthem. Faxed everythng back to Cochlear and they mailed everything to Anthem the day after my translab surgery (May 24th). I was told it would take anywhere from 30 to 60 days for Anthem to make a decision. Fingers crossed ;D

Kixit, I am watching how this goes for you since Anthem (CoVA Care) paid for mine without hesitation. It would seem to me they could only legitimately refuse you based on the coverages in the contract.

Good luck to you, and everyone else!
